Attacks of Opportunity

How do you guys want to deal with Attacks of Opportunity? Currently I'm doing a kind of 'after the fact' method. It works but it's not perfect, for instance an AoO could incapicitate a foe and it's attacks would be worthless as would any other PCs aiming for that foe though I would move their targets to the next nearest one most of the time. Any ideas?
